Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards, and safety standards. Complies with governmental and accreditation regulations.

Provides behavioral health services and clinical case management to the patient population served by the SBHC. Makes treatment recommendations and provides complex services essential to the resolution of social problems which may require the development and adaptation of various treatment approaches, methods, and techniques. This action includes but is not limited to de-escalation, assessing suicidality, and crisis intervention.

Develops and/or collaborates on a series of psycho-educational and social service workshops based on the most pressing needs identified by clinic personnel and families within the school's community.

Identifies appropriate referral sources to facilitate access to community resources and referrals with community-based partner organizations related to child advocacy, mental health, and social services.

Links students/families needing psychiatric care to services available through community network.

Provides professional advocacy skills to both the students and their families.

Works directly with clients to assess health, create plans and evaluate progress.

Collaborates with other health care professionals as part of a multi-disciplinary team. Facilitates communication regarding relevant psychosocial issues of patients and families to health care team through recording in medical records, consultation and participation in relevant meetings.

Functions as a coordinating link between patient, family, school, and community resources to assure continuity of care.

Represents social work perspective and participates in orientation, training, and education programs for students as well as physicians, nurses and other health care professionals as required.

Assumes responsibility for identifying training needs and locating resources and promoting own professional development.

Collaborates with other disciplines in development of new programs that enhance the quality of services to the School Health Center patients and their significant others.

Actively cooperates as a participant in the Division of Community Pediatrics, committee work, research, QA/QI, and other duties as appropriate.

Participates in meetings and on committees and represents the department and hospital in community outreach efforts. Participates in multi-disciplinary quality and service improvement teams.
